About The Position

The Social Worker 3 role within University Hospitals Behavioral Health offers a unique chance to become part of a dynamic, collaborative interdisciplinary team. This position adds value to patient care and significantly assists individuals with various issues. We currently offer an opportunity for a full time, days Social Worker 3 to support patients in Cleveland.

Requirements

  • Master's Degree from an accredited school of Social Work (Required)
  • Computer skills (Required proficiency)
  • Theoretical and clinical knowledge regarding health care delivery (Required proficiency)
  • Multi-tasks and prioritizes work (Required proficiency)
  • Works autonomously (Required proficiency)
  • Communicates effectively with persons of various backgrounds (Required proficiency)
  • Licensed Independent Social Worker (LISW) (Required)

Nice To Haves

  • Hospital/Health Agency experience. (Preferred)
  • Licensed Independent Social Worker Supervisor (LISW-S) (Preferred)
  • Previous experience working with adult patients with various mood disorders, including depression, anxiety, bipolar, etc. is strongly preferred.
  • Proficient in performing diagnostic assessments, treatment planning, documentation, etc.
  • Knowledge of Epic is a plus.

Responsibilities

  • Collaborates with all members of the health care team, patient and family (or significant others) to coordinate timely and efficient assessment of psychosocial and discharge planning needs and formulation of patient and family centered discharge plans.
  • Serves as a resource within the social work department and the hospital.
  • Provides psychosocial assessment / intervention to assist patients/families (including clinically complex).
  • Provides age specific care to patients/families in all age groups.
  • Develops, documents and implements a comprehensive plan based on identified psychosocial needs and/or discharge needs.
  • Develops plans with attention to individual patient and family goals
  • Provides linkage to community resources and UH network services to facilitate optimal care; and serves as liaison to community agencies.
  • Communicates/collaborates with the work team using a multidisciplinary approach.
  • Provides guidance and serves as consultant to staff and other disciplines regarding complex psychosocial and/or discharge planning issues/barriers.
  • Assists with recruitment, and orientation/mentoring/education of new staff and/or social work interns.
  • Performs other duties as assigned.
  • Complies with all policies and standards.
  • Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.
