About The Position

The position will include advanced-level case management and clinical social work services for children and families in the UNC Children’s Complex Care Program. This role functions with minimal supervision and serves as the primary social work lead for the most complex cases. The position will also involve providing integrated behavioral health/counseling services in the UNC Children’s Complex Care Clinic.

Responsibilities

  • Serving as the primary social work lead for children with medical complexity and high acuity psychosocial needs, exercising advanced clinical judgment and independent decision making.
  • Independently conducting comprehensive psychosocial assessments and developing and implementing treatment plans with minimal review.
  • Acting as a clinical consultant to other social workers, care managers, physicians, and interdisciplinary team members regarding assessment, treatment planning, and management of complex cases.
  • Obtaining, interpreting, and communicating complex psychosocial, medical, and social information to clinical teams to support care planning and clinical decision making.
  • Developing and maintaining collaborative relationships with community health, mental health, and social service agencies, and independently identifying and developing resources to meet patient and family needs.
  • Coordinating and organizing complex care across medical, behavioral health, and community systems.
  • Attending Complex Care Clinic (~2 days/week) to assess new patients and provide follow-up for existing patients.
  • Attending multidisciplinary care conferences as the lead social work representative, providing expert input on psychosocial needs and care plans.
  • Leading or independently contributing to the development and implementation of clinical protocols, family assessment tools, and quality improvement for the Complex Care Program.
  • Providing evidence based counseling and behavioral health interventions for patients with complex medical and psychosocial presentations.
  • Modifying therapeutic approaches to address high acuity, chronic, or crisis driven behavioral health needs.
  • Coordinating behavioral health care plans with primary care providers and specialty teams, and providing consultation regarding treatment recommendations.
  • Documenting and billing independently and appropriately for clinical services provided.
