Social Work Care Manager
About The Position
Mosaic Health has an immediate opening for an experienced Social Work Care Manager to join our integrated care team in the Mohawk Valley. This role involves working as a member of a multi-disciplinary treatment team, advocating for patient social needs, accepting warm hand-offs and referrals from providers for patients demonstrating social needs, assessing those needs, and establishing a plan of care. The position also includes linking/referring patients to available community resources/services and assisting with application processes. Additionally, the role requires maintaining Certified Application Counselor status to assist with health insurance program enrollment and helping patients apply for the Mosaic Health Sliding Fee Discount Program. Developing relationships with potential community referral sources and advocating for Mosaic Health services through community outreach and education are also key duties.
