About The Position
Baptist Health is seeking a Social Services Team Lead at their Baptist Beaches campus. This is a Full Time opportunity. The Social Services Team Lead will assess psychosocial needs and provide services that enhance patient functioning, emotional well-being and continued access to services and resources in the community. They will also provide discharge planning, arrange home health and DME, assist with placement/transfers, and provide referrals to community services. Responsibilities
- Assess psychosocial needs and provide services that enhance patient functioning, emotional well-being and continued access to services and resources in the community.
- Provide discharge planning.
- Arrange home health and DME.
- Assist with placement/transfers.
- Provide referrals to community services.
- Serve as a social work resource for team members and the care coordination program.
- Provide guidance through challenging situations.
- Promote problem-solving/critical thinking skills among team members.
- Assist with interviewing and hiring staff members.
- Coordinate orientation and training for newly hired staff through personal mentoring or assigning a mentor, and monitoring progress towards established expectations.
