About The Position

The Social Services Liaison supports residents and families by assisting with communication, resources, documentation, and resident-centered services in collaboration with facility leadership and the interdisciplinary team.

Requirements

  • Previous experience in long-term care, healthcare, customer service, community services, human services, or related environment preferred.
  • Must be able to interact positively and professionally with residents, families, staff, and community agencies.
  • Strong communication, organization, and interpersonal skills.
  • Ability to maintain confidentiality and handle sensitive information appropriately.
  • Ability to work collaboratively with employees at all levels.
  • Must demonstrate compassion, patience, professionalism, and respect for residents and families.
  • Must be able to maintain consistent attendance and be willing to work irregular hours, as needed.
  • Ability to walk, reach, bend, stretch, pull, and perform other physical activities as required to perform job duties.
  • Adequate fine and gross motor coordination to carry out required tasks.
  • Ability to read, write, and speak fluent English to communicate effectively with residents, families, staff, and community agencies.

Responsibilities

  • Serve as a friendly and supportive point of contact for residents and families regarding non-clinical social service needs and general questions.
  • Assist residents and families in identifying available facility resources, community resources, support services, and financial assistance programs.
  • Provide residents and families with information regarding available resources and assist with connecting them to appropriate departments, agencies, or community organizations.
  • Assist with communication between residents, families, facility staff, and community agencies as directed.
  • Participate in resident-centered activities and initiatives designed to support residents' social and emotional well-being.
  • Assist with coordinating family meetings, resident events, and other activities that promote communication and resident engagement.
  • Assist with gathering and maintaining information and documentation related to social service activities, resident concerns, family contacts, and resource referrals.
  • Assist with obtaining information from residents and families and communicate relevant information to the appropriate facility leadership or interdisciplinary team members.
  • Follow up on non-clinical resident and family requests and communicate concerns to the appropriate member of the facility team.
  • Assist with providing residents and families with information regarding facility processes, available services, and community resources.
  • Support resident and family communication during transitions of care by coordinating information and directing questions or concerns to the appropriate member of the care team.
  • Maintain resident confidentiality and treat residents with kindness, dignity, and respect in accordance with Resident Rights.
  • Work collaboratively with nursing, administration, activities, admissions, and other departments to promote a positive resident experience.
  • Assist with maintaining an organized and welcoming Social Services area and resource materials.
  • Participate in facility meetings, training, and quality improvement activities as assigned.
  • Maintain professional relationships with residents, families, staff, and community organizations.
  • Performs other duties as assigned.
