About The Position
This position is responsible for organizing, developing, and coordinating aspects of resident care within a long-term care setting. The role involves conducting initial and ongoing assessments to meet resident needs and serving as a liaison between residents, families, and staff to address any concerns that may arise.
Requirements
- Must be willing to work flexible hours.
- Must have excellent communication skills, written and verbal.
- LSW or a degree in social work required, a degree in similar field with experience on long term care will be considered.
Nice To Haves
- At least one year experience in Social Service work in a long term care setting preferred but not necessary
Responsibilities
- Organizing, developing and coordinating aspects of resident care.
- Initial and ongoing assessments required to meet resident needs.
- Liaison between the resident, family & staff regarding concerns, which may arise.
