About The Position

Tice Valley Post Acute is seeking a compassionate, organized, and proactive Social Services Director to join our interdisciplinary team. In this vital role, you will act as a primary advocate for our residents, ensuring their physical, mental, and psychosocial well-being is maintained at the highest practicable level. • Demonstrated ability to manage sensitive relationships with residents and their families.

Responsibilities

• Evaluate psychological and emotional needs to develop comprehensive care plans.
• Assist residents in achieving independence and self-care goals.
• Provide counseling and support for residents and families coping with physical disabilities, health conditions, grief, and loss.
• Assist with healthcare decisions and coordinate on-site ancillary services including psychiatry, dentistry, podiatry, and optometry.
• Lead discharge planning by coordinating with community agencies, ensuring residents have the necessary equipment and referrals for a safe transition.
• Secure prosthetics, assistive devices, and community resources to meet individual resident needs.
• Document resident status accurately, manage reports of missing belongings, and provide in-service training to facility staff as needed.
• Build strong working relationships with internal departments and outside agencies to ensure seamless service delivery.
