Requirements

• At least one year of social work experience with supervisory duties.
• Direct work experience with geriatric residents.
• A current Missouri or Illinois Social Work license (location dependent).
• Experience with Medicaid and Medicare.
• Sufficient regulatory knowledge to satisfy surveyors.
• Strong leadership skills to guide a team.
• Excellent communication skills.
• Ability to collaborate with physicians and other healthcare professionals.
• Working knowledge of behavioral health systems.
• Networking skills to effectively get things done.
• Successful background check and drug screen.

Responsibilities

• Gathering and assessing information about residents’ psychosocial needs, risk factors, life stories, and quirks.
• Designing and implementing social service interventions to improve quality of life.
• Completing required documentation accurately and professionally.
• Masterminding discharge plans that are sensible and ensure resident safety.
• Leading the social services team with charisma and compassion.
• Coordinating the Family Council.
• Managing the grievance process with grace and professionalism.
• Collaborating with families, physicians, staff, and referral sources.
• Tracking quality indicators related to social services.
