This role is responsible for establishing a course of action by exploring options and setting goals with residents and their families. The position aims to assist residents in achieving the highest practicable level of self-care, independence, and well-being by providing medically related social services. The Social Services Director evaluates social and family information, psychological and emotional needs to assess social services requirements and develop care plans. They provide emotional support, address emotional problems, and assist residents and families with coping mechanisms for physical disabilities, health-related fears, and grief. The role also involves assisting residents with healthcare decisions, personal shopping, and inventory/tracking of patient belongings, including coordinating responses to reports of missing items. Additionally, the position helps secure resources from community and social services agencies, assists in obtaining prosthetics and assistive devices, and develops strong working relationships with other departments and external agencies. Documentation of resident social service status, assistance with outpatient appointments and ancillary services, and coordination of discharge planning with relevant parties and the interdisciplinary team are key functions. The role also includes providing in-service training to staff and counseling residents and family members.
