Requirements

Bachelor's degree in Social Work
Two years of social work experience required
Skilled nursing or healthcare experience required
Knowledge of state and federal regulations
Strong communication, assessment, and documentation skills
Ability to work collaboratively with residents, families, and interdisciplinary teams

Responsibilities

Advocate for residents' social, emotional, and psychological well-being
Support residents and families throughout their stay and discharge planning process
Coordinate care and resources with residents, families, and community partners
Participate in interdisciplinary care planning and resident assessments
Help ensure residents receive the support and services they need to thrive
Promote resident rights, dignity, and quality of life
