About The Position
San Bruno Skilled Nursing is seeking a compassionate and experienced Social Services Director to join our dedicated team. In this role, you will advocate for residents' social, emotional, and psychosocial well-being while collaborating with families, healthcare providers, and community resources to ensure the highest quality of care.
Requirements
- Bachelor's Degree in Social Work, Human Services, or a related field required.
- Minimum of 2 years of supervised social work experience in a healthcare setting.
- Excellent communication, organizational, problem-solving, and interpersonal skills.
- Ability to maintain confidentiality and exercise professional judgment.
Nice To Haves
- MSW preferred.
- Skilled nursing or long-term care experience preferred.
- Experience with PointClickCare (PCC) preferred.
Responsibilities
- Assess residents' social, emotional, and family needs and develop individualized care plans.
- Assist residents in achieving the highest practicable level of independence and well-being.
- Provide medically related social services to support residents' physical, mental, and psychosocial health.
- Offer counseling and emotional support to residents and families coping with illness, disability, grief, and life transitions.
- Coordinate community resources and agency referrals to meet resident needs.
- Assist residents and families with healthcare decisions and discharge planning.
- Coordinate outpatient appointments and ancillary services, including dental, optometry, podiatry, and psychiatric care.
- Maintain accurate and timely social services documentation.
- Collaborate with interdisciplinary team members to ensure comprehensive resident care.
- Assist with resident belongings management and related concerns.
- Provide staff education and in-service training as needed.
- Foster positive relationships with residents, families, staff, and community partners.
