About The Position

We are seeking a compassionate, organized, and resident-focused Social Services Director to join our skilled nursing and long-term care team. This role is ideal for someone who enjoys building relationships with residents and families, supporting individualized care needs, and working closely with an interdisciplinary team to promote each resident’s well-being. Bring experience working with SNF/LTC residents and knowledge of MDS 3.0. BSW preferred, but not required unless required by state regulations. Have at least two years of supervisory experience in a hospital, nursing care facility, or related medical setting.

Responsibilities

  • Communicate clearly and professionally with residents, families, staff, caseworkers, and community partners.
  • Meet with residents and families during the admission process to review advance directives, resident rights, and non-discrimination policies.
  • Complete and document social services evaluations, including identification of resident needs, concerns, and care goals.
  • Coordinate or arrange social work services based on resident and family needs.
  • Assist residents with financial planning needs when appropriate.
  • Keep families informed of resident needs, concerns, transfers, and changes in level of care.
  • Maintain current community resource information to support referrals and discharge planning.
  • Collaborate with facility staff to address resident concerns and support quality outcomes.
  • Serve as a liaison between the facility, families, community resources, and outside agencies.
  • Ensure documentation and services comply with corporate, state, federal, and regulatory standards.
  • Provide social work consultation to residents, families, and staff as needed.
  • Participate in facility meetings, in-service education, continuing education, and quality improvement initiatives.
  • Demonstrate genuine care, compassion, and respect for elderly and disabled residents in a nursing facility setting.
  • Follow Residents’ Rights, facility policies and procedures, and physical demand requirements.
