About The Position

Hearthstone Nursing and Rehabilitation Center, part of the Volare Health network, is seeking an experienced Social Services Director in Medford, Oregon. This role involves advocating for residents, addressing their social, emotional, and psychological needs, and supporting them from admission through discharge. The position requires close collaboration with residents, families, caregivers, and an interdisciplinary team to ensure personalized and compassionate care. Requirements

  • Minimum of 2 years of social work experience; healthcare or long-term care experience preferred.
  • Bachelor’s Degree required.
  • Strong knowledge of social services practices and applicable state and federal healthcare regulations.
  • Excellent written and verbal communication skills.
  • Strong interpersonal skills and the ability to build trusting relationships with residents and families.
  • Ability to work effectively as part of an interdisciplinary healthcare team.
  • Comfortable using computers, electronic documentation systems, and other workplace technology.
  • Strong organizational skills with the ability to manage multiple priorities in a fast-paced environment.
  • A compassionate, resident-centered approach and genuine desire to make a difference.

Responsibilities

  • Complete individualized social histories, assessments, and care plans for residents upon admission.
  • Identify residents’ social, emotional, and psychological needs and establish appropriate goals and interventions.
  • Participate in interdisciplinary care plan meetings and collaborate with the healthcare team to provide comprehensive resident care.
  • Maintain timely and accurate documentation, progress notes, and records in accordance with facility policies and state and federal regulations.
  • Assist residents and families with discharge planning, placement, and transitions of care.
  • Provide guidance, advocacy, and emotional support to residents and their families throughout their stay.
  • Organize and facilitate family meetings and group sessions to promote communication, education, and support.
  • Educate residents, families, and the community about aging, resident rights, available services, and long-term care resources.
  • Assist with pre-admission screening and support prospective residents and families throughout the admission process.
  • Help new residents and their families adjust to the long-term care environment.
  • Maintain accurate social services records and manage department supplies and budget in accordance with company policies.
  • Collaborate with community agencies and outside resources to connect residents and families with appropriate services.
