Requirements
- Bachelor's degree
- experience in SNF
- excellent communication skills
- collaborative approach
- compassion for a vulnerable population
Responsibilities
- Work with residents, their families, physicians, and care team to ensure the resident's needs are met both physically and emotionally.
- Collaborate with admissions and discharge teams to plan accordingly for the resident's care during and after their stay.
- Work under supervision of our Administrator to ensure compliance with current state, federal, and local standards.
