About The Position

Heritage Post Acute is seeking a compassionate and dedicated Social Services Coordinator to join our interdisciplinary team. The ideal candidate is committed to enhancing the quality of life for our residents through exceptional psychosocial care, advocacy, and discharge planning.

Requirements

Master of Social Work (MSW) from an accredited college or university.
Current Licensed Master Social Worker (LMSW) license in the State of South Carolina.
Strong knowledge of long-term care regulations, discharge planning, and resident rights.
Excellent communication, organizational, and interpersonal skills.
Ability to work collaboratively with residents, families, physicians, and interdisciplinary team members.

Nice To Haves

At least one (1) year of experience in a skilled nursing, rehabilitation, or long-term care setting.
Experience with electronic medical records (EMR).

Responsibilities

Conduct comprehensive psychosocial assessments for residents.
Develop, implement, and update individualized social service care plans.
Coordinate discharge planning and transitions of care.
Provide counseling, support, and crisis intervention to residents and families.
Connect residents and families with community resources and support services.
Advocate for residents' rights and promote resident-centered care.
Participate in interdisciplinary care plan meetings.
Maintain accurate and timely documentation in the electronic medical record.
Ensure compliance with CMS, state, and facility regulations.
Collaborate with nursing, therapy, dietary, activities, and other departments to meet residents' psychosocial needs.
Participate in Quality Assurance and Performance Improvement (QAPI) activities.
