A Social Services Coordinator coordinates care planning, psychosocial assessments, admissions, and discharge processes by collaborating with interdisciplinary teams, residents, and families to ensure appropriate services, referrals, and support are in place. Maintains knowledge of community resources, regulatory requirements, and third-party payer programs, while facilitating care conferences, developing resident care plans, and providing education on resident rights, psychosocial needs, and related topics.

Requirements

  • A Bachelor's degree from an accredited college or university in Social Services or Social Work is required.
  • Prior experience working in a group setting.
  • Prior experience working in healthcare.
  • Must be able to work in collaboration with others.
  • Is knowledgeable about Medicare and other third-party payors.
  • Requires good communication and human relation skills.
  • Strong problem-solving skills are necessary with the ability to collect, compare, sort and prioritize information to be used in the decision-making process.
  • Must be able to adapt to unpredictable situations.

Nice To Haves

  • Experience working in post-acute rehabilitation and/or long-term care is preferred.
  • Preferred license by the Minnesota Board of Social Work at minimum a LSW (Licensed Social Worker).
  • Candidates who have recently taken the Minnesota Board of Social Work license test may apply.
  • Candidates who already have a test date scheduled may also apply.
  • One year of work experience in Long Term Care or Home Health is preferred.
