Requirements

• High School Diploma or GED
• 3+ years of experience in services for the elderly, disadvantaged, or social services in healthcare setting

Nice To Haves

• Bachelor's Degree
• 5+ years of experience in services for the elderly, disadvantaged, or social services in healthcare setting

Responsibilities

• Connects residents and home-based non-residents to appropriate supportive services and assists with transitions to higher levels of care when needed
• Monitors the frailest residents through regular contact and post-hospital/therapy transitional care
• Develops and maintains a resource guide, and produces monthly service reports and annual HUD reporting
• Creates communication tools (newsletter, website, social media, marketing materials) and serves as liaison to resident groups
• Coordinates socialization opportunities, including events, classes, spiritual resources, and vocational/educational programming, to reduce isolation
• Works closely with management to ensure residents meet lease obligations and remain in compliance with HUD requirements
