Social Services Assistant
About The Position
Diablo Valley Post Acute is seeking a compassionate, organized, and resident-focused Social Services Assistant to join our interdisciplinary team. In this role, you will help support the emotional, social, and psychosocial well-being of our residents while assisting with the planning, coordination, and implementation of social service programs. This position plays a key role in coordinating services, assisting with discharge planning, maintaining documentation, and helping residents and families navigate healthcare and community resources.
Requirements
- Proficiency with documentation and basic computer systems.

Responsibilities
- Assist residents in achieving the highest practicable level of independence, self-care, and psychosocial well-being.
- Provide support to residents and families coping with illness, disability, transitions in care, and other life challenges.
- Help identify residents' social and emotional needs and assist in developing appropriate service plans.
- Maintain regular social services progress notes and documentation.
- Collaborate with administration, nursing, rehabilitation, and other departments to coordinate resident services.
- Assist with admissions, transfers, and discharge planning.
- Coordinate outpatient appointments and on-site ancillary services, including optometry, podiatry, dental, and psychiatric services.
- Connect residents and families with community resources, healthcare agencies, and financial assistance programs.
- Assist in planning, implementing, and evaluating social service programs.
- Participate in policy development, review, and implementation.
- Support orientation and in-service training programs related to residents' social, emotional, and medical needs.
- Participate in quality improvement initiatives and interdisciplinary meetings.
- Complete required reports, assessments, and documentation accurately and timely.
- Maintain confidentiality of resident information and protected health information (PHI).
- Ensure compliance with federal, state, and facility regulations.
- Report any known or suspected incidents of fraud, abuse, neglect, or unauthorized disclosure of resident information.
