The primary purpose of this job position is to assist in planning, developing, organizing, implementing, evaluating, and directing the facility’s social service programs in accordance with current existing federal, state, and local standards, as well as established policies and procedures, to assure that the medically related emotional and social needs of the resident are met/maintained on an individual basis. This role involves assisting residents in achieving the highest practicable level of self-care, independence, and well-being, and providing medically related social services to attain or maintain the highest practicable physical, mental, and psychosocial well-being of each resident. The position also requires performing administrative duties, providing information on financial assistance programs, and collaborating with staff and community agencies to solve resident needs. A key aspect is working with emotional problems, including assisting residents and families with anxieties and stress related to illness, admission, coping with disabilities, and end-of-life concerns. The role also includes assisting in discharge planning and developing a good working rapport with internal departments and external agencies. Additionally, the position contributes to orientation and in-service training programs and assists in budget development. Maintaining resident care confidentiality and reporting any suspected fraud or unauthorized disclosure of information are critical responsibilities.
