About The Position

We are hiring a Social Services Representative (SSR) to work in the lobby of our largest health center, El Nuevo San Juan/Simpson Pavilion. The SSR provides necessary support services to our patients including medical transportation, home care requests, nutrition/food resources, medical equipment, support services, and other related areas to address the needs of our patients. This is a great opportunity for someone with a High School Diploma who is interested in getting into the field of social services/social work. Must be able to commit to a Monday to Friday 8:00am-5:00pm schedule. The position is fully onsite 5 days/week.

Requirements

• High School Diploma required.
• Ability to work in a high-volume and fast-paced environment.
• Strong customer service skills are required.
• 1 year of experience in healthcare
• Effective oral and written communication.
• Passion for helping the elderly.
• Bilingual English/Spanish is required.

Responsibilities

• Assists providers with providing services including but not limited to: Medical Durable Equipment, Transportation, Food Pantries/Nutrition programs, and Home Care Services.
• Supports completion of forms, generates letters, and requests for services.
• Completes home care requests and prior authorizations.
• Assists with medical record requests including HIPPA forms.
• Supports social workers and case managers with service provisions.
• Assists with the completion and requests of prior authorization at the request of providers.
