This position is primarily responsible for coordinating home and community based services to individuals and families within agency policies, coordinates social programming for the property, and provides social service intervention as needed. The role involves assessing residents' needs, making referrals to community services, providing crisis intervention, case management, and advocating for residents. The coordinator must maintain an up-to-date knowledge base of services impacting the resident population, identify educational and health programs, and build support systems for residents. Responsibilities also include publishing a newsletter, fundraising for programs, demonstrating cultural sensitivity, maintaining resident confidentiality, utilizing social services software, reporting potential abuse or neglect, and participating in property planning and policy development. Communication and coordination with property management staff are essential, as is meeting continuing education requirements for HUD.
