About The Position

A Social Services Coordinator reviews, monitors, and processes a variety of assigned client cases involving the provision of social, health, vocational, rehabilitation, or related services to City residents. This role does not involve supervisory duties. Incumbents receive general supervision from a technical superior and work a conventional workweek. The position may require work in homes, health clinics, and neighborhood communities, with potential exposure to dangerous or unsanitary conditions, hostile individuals or groups, and contagious diseases. The work requires minimal physical exertion.

Requirements

  • A bachelor’s degree from an accredited college or university in vocational rehabilitation, health or human services, nursing or a closely related health or social service field.
  • One year of experience in coordinating, monitoring, and processing cases involving the provision of health, social, vocational, or rehabilitation services to clients.
  • Equivalent combination of education and experience.
  • Authorization for a Criminal Background Check and/or Fingerprint must be successfully completed for positions of trust.
  • Completion of a mandatory six-month probation period.
  • Education must be obtained from an accredited institution.
  • Education credentials obtained out of the US must be evaluated for US equivalency.

Responsibilities

  • Reviews, monitors, and processes assigned client cases.
  • Provides social, health, vocational, rehabilitation, or related services to City residents.
  • Maintains confidential, accurate, and detailed client case information and records.
  • Conducts and processes multiple case assignments simultaneously.
  • Motivates others to follow a designated regimen or course of action to effect behavior modifications.
  • Reviews and analyzes client records to evaluate the implementation and progress of recommended care or strategies.
  • Ensures progress, recovery, or treatment complies with professional standards.
  • Coordinates responses from multiple public and private agencies, institutions, and community resource organizations.
  • Collects, reviews, and analyzes client case records to make recommendations on simplifying reporting and documentation practices and procedures.
  • Communicates effectively, both orally and in writing.
  • Disseminates information and explains City, State, and private institutional services and programs to individuals, businesses, and community groups.
  • Establishes and maintains effective liaisons with people of varied backgrounds, including clients, families, neighbors, government representatives, and public and private community institutions, service providers, and personnel.
  • Deals with emergency or crisis situations with calm, tact, and dispatch.
  • Operates and utilizes standard business PC equipment and software programs.
