As a member of the Learning Community Center of South Omaha’s (LCCSO) Family Engagement team, the Social Assistance Navigator connects families across all program sites with vital social services and community resources. This role requires applying professional human services principles and techniques to help participants navigate challenges and improve their overall well-being. Visits participants in their homes and assesses the needs of participants and their families to connect them with community resources. Applies the principles and practices of social work in the performance of assignments such as assessing, developing, and providing direct services or programs to individuals, families, and/or groups. Serves as point of contact for participants who present with any type of abuse or neglect. Conducts screenings and education for participants related to their health and well-being. Updates family’s progress through the database, adding relevant notes, service plans, discharge plans, files, evaluations, visits, etc., in a timely manner. Helps parents overcome any barriers to fully engaging in the program. Uses effective interpersonal skills in working with and receiving referrals from the various teams of the Learning Community Center of South Omaha.
