About The Position

The Patient Navigator - Social Service Worker will help patients have access to the full continuum of cancer care from screening to detection, diagnosis, treatment, and resources, available beyond the completion of treatment. This position focuses on resource coordination, barriers to care, and supportive interventions to improve patient access, treatment adherence, and overall well-being. This social services navigator collaborates with interdisciplinary team members to identify patient needs and connect patients with internal, community, and philanthropic resources including transportation, lodging, medication assistance, durable medical equipment and home support resources, food insecurity programs, financial support, wigs/prosthetics, support groups, and other cancer-related supportive services.

Requirements

  • Bachelor's Degree in Family and Consumer Science, Social Work, Sociology, Psychology, or a related field required.
  • Strong communication, assessment, and care coordination skills; knowledge of healthcare systems and resources.
  • Must possess excellent time management skills and be able to efficiently prioritize work duties.

Nice To Haves

  • 3-5 years Clinical experience in healthcare or social services settings is preferred.
