Join East Bay Community Action Program (EBCAP) as a Smart Optimal Solutions (SOS) Coach and play a meaningful role in supporting individuals and families on their path toward self-sufficiency. In this rewarding position, you'll provide personalized coaching, connect clients with valuable community resources, and help create positive outcomes that strengthen our communities.
As a Smart Optimal Solutions Coach, you will work closely with participants in the LIHEAP Program, providing one-on-one assessments, ongoing case management, and personalized support. You'll help individuals and families identify goals, improve financial stability, learn energy-saving practices, and connect with workforce development opportunities and other community resources.
This position also serves as an important link between EBCAP, community partners, and participants by supporting outreach efforts, developing educational workshops, tracking program data, and helping ensure services are delivered effectively and efficiently.

• Knowledge of community resources, social service systems, and available support programs.
• Ability to connect individuals and families with services that best meet their needs.
• Ability to travel between program sites and community locations.
• Ability to establish trust with clients while maintaining professional boundaries.
• Ability to collaborate effectively across programs and service providers.
• Ability to complete assessments, coordinate referrals, and monitor client outcomes.

• Bachelor's Degree in Human Services or a related field preferred.

Responsibilities

• Conduct comprehensive assessments and provide individualized case management services to eligible households.
• Support clients with energy conservation education, financial literacy guidance, and workforce development referrals.
• Build strong relationships with clients while maintaining a strengths-based, person-centered approach.
• Coordinate services and referrals with internal programs and external community partners.
• Assist individuals with resource eligibility determination and support the application process for LIHEAP and related services.
• Monitor client progress, conduct follow-up activities, and update service plans as goals evolve.
• Maintain accurate, complete, and timely case documentation and data entry.
• Prepare reports, statistics, and program-related documentation as required.
• Develop and facilitate workshops and educational opportunities for participants.
• Represent the agency at community outreach events and monthly DHS meetings.
• Collaborate with program leadership on quarterly reporting and program improvement initiatives.
• Participate in trainings, team meetings, and professional development opportunities.
• Perform additional duties as needed to support program success.
