About The Position

The Ward Clerk works in conjunction with Providers, nursing staff, residents, families, and all facility departments to enhance optimal functioning of care delivery and departmental operations. They are responsible for all clerical and receptionist duties at the nursing station and support the Director of Nursing (DON) or designee. This job description is intended to identify some of the primary duties and responsibilities. Mayers Memorial Hospital District (MMHD) reserves the right to modify, supplement, delete or augment the duties and responsibilities specified in this position description, at MAYERS MEMORIAL HOSPITAL DISTRICT’S sole and absolute discretion.

Requirements

  • High school graduate or equivalent.
  • One to three years’ related experience and/or training.
  • Experience in a long-term care setting preferred.

  • Assists DON (or designee) with survey readiness and compliance documents.
  • Uploads documents into Point Click Care (PCC) as needed.
  • Use appropriate telephone and paging system etiquette.
  • Answers the telephone with a greeting, identifies self, department and relays messages promptly and accurately, places call and/or pages as requested.
  • Greets internal and external customers in a friendly and courteous manner, assisting in directing them appropriately and/or meeting their needs as much as able.
  • Assists with enforcing visiting policies, guidelines, and regulations.
  • Maintains Periodic Automatic Replacement (PAR) levels for forms, materials, and supplies by requisitioning items in a timely manner and according to policy.
  • Maintains a responsive, clean, and hazard-free work area.
  • Assist in maintaining a professional environment.
  • Prepares various SNF charts packets.
  • Assuring that all packets have current forms and are comprehensive/complete.
  • Maintains SNF charts using current forms and removing excess forms according to procedure.
  • Inserts adequate forms for 24-hour coverage.
  • Initiates admission charts with residents’ information, complete headings, correct ID number and complete ID band.
  • Checks all admission papers for omissions and flags to alert discipline accordingly.
  • Scheduling of resident appointments.
  • Prepares (and maintains) discharge/transfer documents for state, residents and family’s communication and compliance.
  • Makes out requisition forms for all ancillary departments accurately and in a timely manner.
  • Notifies these departments of orders and places reports in the chart after review by the charge nurse in the correct manner.
  • Ensures Level of Care Cards at resident bedside remains current.
  • Follows discharge procedure to include making follow-up appointments, assuring that appropriate information is included on the discharge instruction sheet, that medication and valuables have been returned to the residents and all designated departments are notified of the discharge.
  • Completes the discharge chart audit for SNF charts according to policy.
  • Check the incomplete chart file regularly to assure that all staff members have completed the sections and alert medical records of the discharge.
  • Assist with resident transfer by making copies of chart forms and assure that transfer papers are completed and make follow-up phone calls if requested to do so.
  • Interfaces well with all other departments in the facility to maintain a quality image of nursing and service.
  • Knowledgeable about responsibilities during emergency situations, fire, safety, and disaster procedures.
  • Performs additional duties as requested.
  • Adheres to dress code; appearance is neat and professional.
  • Completes annual education requirements if applicable.
  • Maintains regulatory requirements.
  • Wears identification while on duty.
  • Attends annual evaluation and participates actively in this process.
  • Reports to work on time and as scheduled; completes work in designated time.
  • Attends meetings as appropriate.
  • Exhibits the mission, ethics, and goals of Mayers Memorial Hospital District in the performance of job duties.
