About The Position

The Shelter Case Manager plays a key role in supporting residents by assessing needs, developing individualized service plans, and connecting clients to housing, medical, mental health, and employment resources. This position focuses on empowering individuals toward stability and self-sufficiency while maintaining accurate documentation, ensuring compliance with program requirements, and collaborating with a multidisciplinary team to provide comprehensive, client-centered care. As a Case Manager at Elmcor, you serve as a central advocate and coordinator for residents—playing a critical role in assessing needs, developing individualized service plans, and guiding individuals toward stability and self-sufficiency. Requirements

  • BS/BA Degree in a human services related discipline and 3-5 years’ experience working with applicable populations.
  • Experience working with unhoused populations
  • Demonstrates strong interpersonal, communication, and problem-solving skills with sound judgment, discretion, and the ability to manage multiple priorities and deadlines
  • Maintains a professional, courteous, and customer service–oriented approach with a positive attitude and attention to detail
  • Remains calm under pressure, able to identify and resolve routine issues effectively in fast-paced or high-stress environments
  • Works independently and collaboratively, fostering a professional and supportive environment for staff, clients, and stakeholders

Responsibilities

  • Develop and monitor individualized service and treatment plans, including regular resident check-ins, assessments, and crisis intervention as needed
  • Connect residents to community resources, social supports, and essential services that promote independence and well-being
  • Coordinate and track services, including follow-ups with referrals and ongoing communication with providers
  • Maintain accurate documentation, resident records, and provide timely reports to supervisors
