About The Position

Columbus House opened its doors in 1982 to provide services for men and women at least 18 years of age. Our goals quickly broadened from the mere provision of food and shelter and simple survival, to understanding and working toward overcoming the problems which cause people to become homeless. Columbus House, along with its core of loyal supporters, friends, and volunteers, has remained committed to fulfilling its mission: "To serve people experiencing homelessness or at imminent risk by providing life-saving outreach, shelter and housing and by fostering their personal growth and independence. We advocate for and create affordable housing to end homelessness in our communities."

Requirements

  • Bachelor’s degree or Associates degree with 3 years relevant work experience required
  • Must have full understanding of and able to effectively use the local network of substance abuse, mental health, and social services providers.
  • Familiar with criminal justice and able to engage, manage, motivate, and support.
  • Time management skills and computer literate with a deadline-oriented mindset.
  • Able to meet all documentation requirements.
  • Complete knowledge base of all aspects of the delivery of person-centered case management.
  • Able to assess strengths, barriers, and needs to assist clients in identifying personal goals, values and priorities, ability to advocate for consumers.
  • Familiar with mental health, substance abuse and dual diagnosis issues and group dynamics; capacity to work within a team and foster teamwork.
  • Flexible, able to seek out opportunities for clients to lead fulfilling lives.
  • Crisis intervention skills, boundaries, confidentiality, universal precautions, harm reduction, strong empathetic skills.
  • Ability to maintain her or his own wellness and to be continually striving for improvement.
  • Valid Driver’s License

Nice To Haves

  • Bi-lingual English/Spanish preferred.

Responsibilities

  • Case manages 25 shelter residents.
  • Provide front door assessments and diversion as part of the Coordinated Access Network.
  • Service and engage consumers weekly at minimum or as needed, building a trusting working relationship.
  • When appropriate, make referrals to agencies for SA/MH, medical, vocational, educational, or housing needs.
  • Link consumers with entitlement providers as needed.
  • Occasionally provide transportation to medical/clinical appointments using agency provided vehicles.
  • Network with other providers that may be working with the consumer.
  • Complete all required documentation accurately and timely, including reporting all contacts and maintaining consumers’ charts.
  • Input all required client-related data into computer database program completely, accurately and in timely fashion.
  • Participate in staff meetings, training, individual supervision, and other related meetings.
  • Facilitate weekly group meetings addressing relapse prevention and life skills in addition to a weekly house meeting.
  • Advocate for client in regulatory and legal settings as required.
  • Ensure random weekly toxicological tests on consumers. Other related duties as assigned.
