As a Case Manager at Elmcor, you serve as a central advocate and coordinator for residents—playing a critical role in assessing needs, developing individualized service plans, and guiding individuals toward stability and self-sufficiency. Requirements
- BS/BA Degree in a human services related discipline and 3-5 years’ experience working with applicable populations.
- Experience working with unhoused populations
- Demonstrates strong interpersonal, communication, and problem-solving skills with sound judgment, discretion, and the ability to manage multiple priorities and deadlines
- Maintains a professional, courteous, and customer service–oriented approach with a positive attitude and attention to detail
- Remains calm under pressure, able to identify and resolve routine issues effectively in fast-paced or high-stress environments
- Works independently and collaboratively, fostering a professional and supportive environment for staff, clients, and stakeholders
Responsibilities
- Develop and monitor individualized service and treatment plans, including regular resident check-ins, assessments, and crisis intervention as needed
- Connect residents to community resources, social supports, and essential services that promote independence and well-being
- Coordinate and track services, including follow-ups with referrals and ongoing communication with providers
- Maintain accurate documentation, resident records, and provide timely reports to supervisors
