About The Position

The Shelter Case Manager provides comprehensive case management and supportive services to clients experiencing homelessness and living in One80 Place Shelter. The primary goal is to assist participants to create a housing plan to locate permanent housing as quickly as possible and ensure participants have a way to maintain housing with targeted referrals to services, resulting in greater self-determination. Functions performed include housing location, housing stability planning, connection to community resources, increase income, healthcare connections and housing problem solving. One80 Place ends and prevents homelessness throughout South Carolina with offices located in Charleston and Columbia. This position will be based in Charleston.

Requirements

Bachelor’s degree in social work, psychology, human services, or related field preferred (or equivalent experience).
Minimum 2 years’ experience working with vulnerable populations, particularly those impacted by homelessness.
Strong verbal and written communication, de-escalation, and relationship-building skills.
Ability to work independently, manage time well, and adapt to crisis situations.
Familiarity with housing programs, local resources, and HMIS is a plus.
Valid driver’s license and reliable transportation.
Excellent computer skills include basic office programs and ability to learn and utilize necessary database systems (Homeless Management Information System).

Responsibilities

Meet weekly with shelter clients to assess needs and monitor progress.
Conduct comprehensive assessments to support participants in improving overall well-being and achieving housing stability.
Facilitate access to behavioral health and medical care as part of the case management process.
Ensure all clients receive information about available medical, mental health, and substance use services through community providers.
Assist with connections and enrollment to targeted referrals and support services.
Assist participants with medically complex needs navigate an appropriate housing plan.
Assist participants in completing the Housing Barriers Assessment and Housing Preference Worksheet and develop housing plans tailored to each client’s goals and housing needs.
Connect participants to supportive services and mainstream benefit programs to promote stability and self-sufficiency.
Refer to available permanent housing programs, when appropriate, to help them secure permanent housing.
Initiate the housing location process.
Intentionally engage program participants to support recovery and address relapse risks, ensuring consistent connection throughout their participation in shelter services.
Recognize and respond to the impact of recovery and relapse on housing stability, adjusting service strategies as needed.
Incorporate recovery-oriented approaches into housing plans to minimize the effects of substance use.
Collaborate with Shelter Assistants and supportive service staff to keep participants engaged in the program and maintain housing upon program exit.
Maintain accurate, timely documentation in HMIS in compliance with agency and funding requirements.
Maintain compliance with program requirements and One80 Place policies and procedures.
Effective use of Outlook calendar for productivity and safety.
Participate in team meetings, case conferences, and staff meetings.
Partner with community providers and housing program staff.
Contribute to program development, evaluation, and continuous quality improvement.
Attend relevant training on topics related to working with unhoused populations.
Ability to foster a “warm hand off” and close collaboration with other case managers or support staff.
