SF Case Manager Manassas
About The Position
Moms In Motion is hiring a Service Facilitator Case Manager to support families navigating Virginia Medicaid Waiver programs, including CCC+, CL & FIS, and EPSDT. The role focuses on helping clients live safe, healthy, and independent lives through home visits, care planning, assessments, and practical support for families. The position requires living in Virginia and includes daily local travel to clients’ homes. Work is also performed remotely using a laptop or tablet. Client transportation is not required. Anticipated annual earnings generally range from $55,000 to $100,000 based on completing a minimum of 125 visits per month; actual earnings vary based on the number and type of visits completed.
Requirements
- At least 2 years of experience supporting individuals with disabilities or older adults.
- Live in Virginia.
- Have a valid driver's license and reliable vehicle; transporting clients is not required.
- Be comfortable using email, documents, spreadsheets, portals, and video calls.
- Have strong written and spoken communication skills.
- Pass a background check and provide 2 professional references.
