About The Position

Moms In Motion is hiring a Service Facilitator Case Manager to support families navigating Virginia Medicaid Waiver programs, including CCC+, CL & FIS, and EPSDT. The role helps clients live safe, healthy, and independent lives by connecting families with resources, preparing plans of care and assessments, and troubleshooting service authorizations and timesheets. The position requires daily local travel to client homes and remote work using a laptop or tablet. Applicants must live in Virginia. No degree is required.

Requirements

At least 2 years of experience supporting individuals with disabilities or older adults.
Live in Virginia.
Be comfortable using email, documents, spreadsheets, online portals, and video calls.
Have a valid driver’s license and a reliable vehicle; transporting clients is not required.
Have strong written and verbal communication skills.
Pass a background check and provide two professional references.
