About The Position

The Service Coordinator is responsible for assessing and verifying service coordination needs, planning services, making referrals, and monitoring service provision. This role involves advocating for consumers, providing after-hours crisis services, and documenting progress. The position requires frequent contact with consumers, family members, and service providers. The candidate must be able to accommodate consumer and family needs, which requires working a flexible schedule, including after-hours, holidays, and weekends.

Requirements

• Bachelor's degree or advanced degree from an accredited college or university (any field)
• Associate degree in a social, behavioral, human service, or health-related field including psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, and criminal justice with at least one year paid or unpaid experience working with individuals with IDD is preferred
• High School diploma or a certificate recognized by a state as the equivalent of a high school diploma, and two years of paid or unpaid experience with individuals with intellectual or developmental disabilities
