Service Coordinator I
About The Position
The Community Access Service Coordinator manages processes for enrollments and transitions for participants in the Texas Home Living and Home and Community Based Services programs. The Service Coordinator works under the supervision of the Community Access Team Leader, IDD Authority Services Division. The IDD Service Coordinator is responsible for performing processes for waiver enrollments, transfers and reassignments, provision of Medicaid billable targeted case management activities (assessment, service planning, monitoring, advocacy and crisis prevention and management for individuals with IDD), compliance with applicable local, state and federal rules and regulations, adherence to and promotion of the Texana Compliance Policy, and travel within a 6-county region. Work hours are Monday-Friday, 8 AM to 5 PM with flexibility based on client need.
Requirements
- Bachelor’s or Advanced degree from an accredited university or college
- Texas Driver License (in good standing)
- No experience required.
Nice To Haves
- Strong organizational and computer skills (Microsoft Office and Electronic Health Record system).
- Knowledge of Service Coordination State Rules.
- Experience working with individuals with IDD.
Responsibilities
