About The Position

As a Service Coordinator (SC) with Alleghenies United Cerebral Palsy (AUCP), you will work closely with individuals and their support teams to develop, update, and manage personalized service plans. Your goal is to ensure that each person receives the support they need to thrive-whether through paid providers or community resources. This is a client-centered role that requires strong communication, coordination, and compliance skills. This is a hybrid position requiring both in-office and remote work. When working remotely, employees must maintain reliable high-speed internet access to ensure consistent connectivity for meetings, systems, and communication. Failure to maintain adequate internet service may impact the ability to perform essential job functions. We also talked about addressing outside employment with new hires earlier in the hiring process. This will help us identify and prevent any potential conflicts of interest and ensure expectations are clear upon acceptance of employment. Additionally, we confirmed that all newly hired employees must have reliable high-speed internet at home to ensure they can access required systems and perform their job duties effectively. Employment is contingent on the successful completion of the following clearances: PA State Police Background Check (e-PATCH), Child Abuse Clearance, FBI Background Check, MA Fraud Check, Social Security Verification, DMV Check.

Requirements

  • Bachelor's degree in Social Work, Psychology, Education, Human Services, or a related field.
  • At least three (2) years of professional experience in case management for people with physical disabilities.
  • Three (2) years of experience working with individuals with Autism
  • Experience working with individuals with disabilities or special needs.
  • Strong skills in assessment, planning, and participant-centered service delivery.
  • Excellent organizational, communication, and documentation skills.
  • Ability to work effectively as part of an interdisciplinary team.
  • Familiarity with community support systems, housing resources, health care, and human services.
  • Must live in PA or be willing to travel to PA to complete home visits
  • Must maintain reliable high-speed internet access.

Nice To Haves

  • Knowledge of Medicaid Waiver programs, Long-Term Services and Supports (LTSS), and Home and Community-Based Services (HCBS) strongly preferred.

Responsibilities

  • Collaborate with clients and their teams to develop individualized service plans (ISP) based on personal goals and priorities.
  • Regularly update service plans to reflect life changes and current support needs.
  • Coordinate and monitor services with paid providers and community resources to ensure they are: Meeting the individual's needs, Authorized and used appropriately, Implemented by knowledgeable staff following the plan.
  • Maintain open communication with support teams to address concerns and updates promptly and effectively.
  • Ensure all work complies with program regulations and AUCP standards, including documentation, timelines, and billing requirements.
  • Travel within the assigned region to meet with participants in compliance with ISP.
