Once determined eligible, we utilize a Person-Centered approach addressing the health and safety needs for the individuals and families being served. We assist individuals and families to identify natural and community supports, potential supports in their own homes, and potential Medicaid-funded supports in the community. We encourage self-determination and offer a supported decision-making process for adults seeking services. Families with children seeking services will receive information on supports available within their communities.

Requirements

  • Managing and facilitating access to essential supportive services within the community.
  • Providing comprehensive case management services tailored to individual needs, ensuring that all clients receive the support they require.
  • Developing innovative programs and resources that promote wellness for individuals with Intellectual and/or Developmental Disabilities, fostering an environment where they can thrive.
  • Enhancing the quality of life for clients through expertise and empathy.
