The Sepsis Coordinator will facilitate, plan, implement, and evaluate a sepsis program, including all systems necessary for a multidisciplinary hospital-based sepsis team. This role will utilize the electronic medical record to identify septic patients, facilitate data collection, monitor and analyze outcomes, and integrate performance improvement activities. The coordinator will also remain abreast of relevant regulations and guidelines, round on septic patients, collaborate with clinical teams on sepsis bundles and best practice plans of care, and address barriers to care delivery. Additionally, the role involves monitoring physician adherence to sepsis bundles, attending Sepsis Rapid Responses, coordinating multidisciplinary services, and representing the organization in meetings and committees. The Sepsis Coordinator is also responsible for developing and overseeing sepsis-related education for new and current team members, providing case studies and patient outcomes, attending stakeholder meetings, and providing ad-hoc in-services. The role also involves utilizing a Nursing Research approach to improving outcomes, with a base knowledge of IRB requirements, and participating in multidisciplinary committees focused on advancing nursing research and professional development.
