Join our multidisciplinary kidney transplant team! The Transplant Social Worker will support kidney transplant recipients and living donors throughout their journey, providing care and guidance both before and after transplant or donation. This role requires the ability to apply the knowledge and skills of advanced social work practice, responding to suspected abuse, neglect or violence in accordance with the National Association of Social Work Code of Ethics policies and procedures alongside the appropriate state laws. The position involves expert-level critical thinking and in-depth psychosocial assessments, with the ability to apply complex concepts to situations. If holding the correct licensure, the role may involve diagnosing patients/clients/residents and providing specialized counseling and support. The Social Worker will be a member of the interdisciplinary team, assisting with social, emotional, and economic concerns of patients/clients/residents and their families/caregivers to help them achieve or maintain an optimal level of functioning. This includes utilizing age-related patient/client/resident care protocols for various age groups and potentially working with clinical staff on fiscally sound and timely continued planning outcomes. The role requires advanced knowledge of teamwork principles, understanding the impact of illness, and possessing knowledge and clinical experience in the psychosocial and economic management of illness-related problems, with the possibility of providing brief therapy. The position also requires the ability to work with the growth and development needs of patient/client/resident populations.
