About The Position

MediLodge of Haggerty Road is seeking a Senior Social Services Director to join their team. This role involves assessing the social and economic aspects of resident care, identifying changes in mood and behavior, and ensuring continuity and coordination of resident care. The position requires strong communication and interpersonal skills to interact with residents, families, and community resources. The director will also be responsible for planning and implementing social services with minimal supervision and maintaining accurate documentation.

Requirements

  • Bachelor's or Master's degree in social work.
  • At least three to five years of experience in a social service program for the elderly or related field.
  • Licensed (full or limited) as a bachelor's or master's level social worker with the State of Michigan.
  • Knowledge of skills necessary to conduct and evaluate assessments of the social and economic aspects of resident care.
  • Knowledge of skills necessary to identify and evaluate changes in mood and behavior that affect residents' lives.
  • Working knowledge of skills necessary to provide continuity in and coordination of resident care.
  • Skills to interview residents and their families.
  • Ability to communicate with community resources.
  • Knowledge of community agencies and other resources for making referrals.
  • Basic business computer knowledge.
  • Ability to communicate effectively with staff, Residents, and families.
  • Ability to plan, implement and work with only minimal direct supervision.
  • Ability to write clearly and concisely in preparation of social histories and care plans.
  • Good listener with the ability to display understanding and a helpful attitude.
  • Valid driver's license.
  • Reliable transportation.

Responsibilities

  • Conduct and evaluate assessments of the social and economic aspects of resident care.

  • Provide continuity and coordination of resident care.
  • Interview residents and their families.
  • Communicate with community resources when necessary.
  • Make referrals for family and resident problems.
  • Plan, implement, and work with minimal direct supervision.
  • Write clearly and concisely in preparation of social histories and care plans.
  • Listen effectively and display understanding and a helpful attitude toward the problems and concerns of residents with terminal illnesses and their families.
  • Attend court hearings and meetings at off-site locations.
