Snowline Health is seeking an experienced, compassionate Social Services Director to lead our social services programs and support our interdisciplinary hospice care team. This leadership role offers the opportunity to guide program development, support staff, and ensure exceptional psychosocial care for patients and families navigating serious illness, grief, and end-of-life care. This role ensures delivery of high-quality psychosocial care that aligns with patient goals, promotes dignity, and supports families throughout the care continuum.

Requirements

  • Master of Social Work (MSW) required
  • Minimum of 5 years of clinical experience in healthcare, hospice, or related field
  • Minimum of 3 years of leadership or supervisory experience
  • Experience supporting patients and families with grief, loss, and end-of-life care
  • Strong leadership, communication, and interpersonal skills
  • Ability to work effectively with diverse populations

Nice To Haves

  • Licensed Clinical Social Worker (LCSW) preferred
  • Advanced certification (e.g., ACHP-SW) preferred
  • Experience with program development preferred

Responsibilities

  • Provide leadership and oversight of social services programs and staff
  • Supervise, mentor, and support social workers and related team members
