Social Services Director
About The Position
The Social Services Director at White Oak Manor Charlotte will lead and oversee the Social Services Department to support residents’ emotional and psychosocial well-being. This role involves developing and implementing individualized care and discharge plans, partnering with residents, families, and the care team to ensure needs are met, and providing guidance, support, and crisis intervention. The director will also coordinate referrals to community resources, ensure compliance with regulations, maintain accurate documentation, supervise staff, and participate in interdisciplinary team meetings and quality assurance initiatives.
Requirements
- Bachelor’s Degree in Social Work or related Human Services field
- At least one (1) year of social services experience in a healthcare setting
- Strong communication and interpersonal skills
- Ability to lead, organize, and prioritize in a fast-paced environment
- Knowledge of regulatory requirements and resident rights
- Compassion, empathy, and a commitment to quality resident care
Responsibilities
