About The Position

This position performs professional, advanced, and specialized services including therapeutic interventions and crisis management for clients’ intricate needs and compounded psychosocial factors. They will manage cases with myriad complexity to include family situations, health/mental health factors, and substance abuse issues in areas related to Indian child welfare, elder and vulnerable adult cases, wraparound supportive services, emergency assistance, and juvenile justice. The incumbent must provide advocacy, linkages to other appropriate services and counseling to address complex emotional and social issues. This position requires frequent travel throughout Southwest Michigan and additional travel throughout Michigan may also be required.

Requirements

  • Master’s degree in a human services field such as social work, mental health, counseling, or psychology.
  • Professionally licensed in degreed field and in good standing in the State of Michigan.
  • Limited licenses must be able to achieve full licensure withing twelve months from the date of hire or promotion into the position.
  • Three years’ experience in case management or degreed field.
  • Valid Michigan driver’s license with good driving record; GSA certifiable.
  • Travel is required.
  • Complete and pass a comprehensive background investigation and drug screening in accordance with NHBP standards.
  • Experience which demonstrates computer proficiency, including email, internet, word processing (Microsoft Outlook) and spreadsheets (Excel) and knowledge of other office equipment.
  • Experience with public assistance and social services systems.
  • Experience working with highly confidential and sensitive information and both preserving and maintaining confidential and sensitive information, which includes following HIPAA requirements.

Responsibilities

  • Conduct comprehensive biopsychosocial, risk, safety and needs assessments for individuals and families in a culturally sensitive manner to identify needs and strengths.
  • Create an individualized, comprehensive, outcome-based and family-oriented service plan that supports clients goals and overall well-being based on a person-in-environment perspective.
  • Monitor and evaluate clients’ progress towards achieving their service plan goals.
  • Provide direct clinical interventions, including brief individual and family counseling to address psychosocial issues.
  • Identify, access, and mobilize community and family resources to address identified needs, such as housing, food assistance, mental health services, substance abuse services, home health support, parenting support, and school advocacy.
  • Act as a central point of contact, facilitating care across the continuum by coordinating with other providers and agencies to arrange care and resources.
  • Manage a caseload of clients, including maintaining detailed, accurate, and confidential client records and contact logs involving individuals and families working with tribal and state agencies to ensure culturally relevant services and documenting efforts and interventions.
  • Respond to allegations of child and elder abuse and neglect in accordance with tribal, state and federal laws including but not limited to partnering with another entity to conduct the investigation.
  • Manage client transitions from levels of care to ensure safe and appropriate planning including discharge planning.
  • Provide direct counseling and therapeutic support to help clients cope with personal and social challenges.
  • Assist clients in understanding their rights and navigating service systems.
  • Facilitate community engagement, outreach, educational and psychoeducational workshops integrating cultural healing traditions.
  • Actively participate in community-building initiatives.
  • Provide crisis intervention, emotional support, counseling and education to help clients and families to cope with life challenges.
  • Prepare thorough biopsychosocial reports, attend, and testify in tribal and state court proceedings to advocate for the best interest of the tribe and its members.
  • Implement services to facilitate family preservation and reunification, conduct home studies, and manage child placements.
  • Advocate for clients’ rights and needs, connect them with resources, remove barriers to services, and address systemic barriers to care within and outside the tribal system.
  • Collaborate with multidisciplinary services ensuring coordination, communication and accountability for client’s services.
  • Develop and maintain collaborative relationships with tribal, county and state service providers.
  • Maintain accurate, thorough, confidential, and timely documentation related to client care, progress and outcomes utilizing SSD electronic record system.
  • Travel to perform job functions and work flexible and varying hours including nights and weekends on a regular basis and as needed.
