About The Position

Dorchester Seniors, Inc. (DSI) is Dorchester County's premier aging services organization, serving adults age 50+ through innovative programs that promote healthy aging, independence, and dignity. We believe every older adult deserves access to the resources, support, and advocacy needed to thrive. We're looking for a compassionate, resourceful, and mission-driven professional to join our team as a Senior Services Care Manager. As the Senior Services Care Manager, you will serve as the primary advocate and care coordinator for older adults experiencing complex health, social, financial, housing, caregiver, or functional challenges. This role goes beyond traditional case management by providing comprehensive, wraparound support that empowers seniors to remain safe, healthy, and independent in their homes and communities. Requirements

  • Associate's degree in Social Work, Human Services, Psychology, Counseling, Gerontology, Public Health, or a related field.
  • Experience in case management, care coordination, healthcare, or aging services preferred.
  • Excellent communication, organizational, and problem-solving skills.
  • A passion for serving older adults with dignity, compassion, and respect.
  • Ability to work independently while collaborating effectively with a multidisciplinary team.
  • Valid South Carolina driver's license and reliable transportation for community and home visits.

Responsibilities

  • Conduct comprehensive member assessments and develop individualized care plans.
  • Coordinate services with healthcare providers, hospitals, home health agencies, and community partners.
  • Connect members with benefits, housing resources, transportation, nutrition programs, and financial assistance.
  • Provide crisis intervention and advocacy for vulnerable older adults.
  • Conduct home visits to assess safety and identify unmet needs.
  • Assist grandparents raising grandchildren through DSI's Kinship Care initiatives.
  • Monitor member progress, provide ongoing follow-up, and evaluate outcomes.
  • Collaborate with DSI departments including Prevention, Community Access, Rural Services, Home Care, and Wellness.
  • Build strong partnerships with healthcare systems, social service agencies, and community organizations.
