We are currently seeking qualified candidates for the following position: Position: Senior Case Manager. In accordance with Division of Mental Health and Addictions Services (DMHAS), Commission on Accreditation of Rehabilitation Facilities (CARF), Medicaid, and Volunteers of America policies and standards, the Senior Case Manager is responsible to deliver wellness and recovery-oriented case management and support services to service recipients. The Senior Case Manager provides support directly to the residents, including but not limited to: skill development, supportive counseling, transportation, crisis intervention, medication monitoring, assistance with task completion, vocational and social support. The Senior Case Manager actively supports the Residence Manager/Director in the management of the program’s staff and resident treatment. The Senior Case Manager is available to cover open shifts when necessary to ensure adequate coverage at the facility at all times. Our program serves consumers with psychiatric disabilities living in their own apartments in a senior housing complex.
