About The Position

The Senior Registered Nurse provides acute, hospital-level care to patients in their homes through Parkland’s Hospital at Home program, which combines in-home care with remote monitoring. The program serves patients with conditions such as pneumonia, cellulitis, COVID-19, urinary tract infection, dehydration, and exacerbations of chronic conditions including heart failure and chronic obstructive pulmonary disease. The RN coordinates patient care in the home using the nursing process and Parkland’s nursing professional practice model: assessing patients in their environment, identifying nursing diagnoses, developing and implementing individualized plans of care, and evaluating patient responses. The RN helps patients move through the healthcare encounter without preventable complications or delays and delegates interventions in accordance with the Texas Nursing Practice Act, patient condition, and staff competencies. The position requires independent judgment and coordination of care for multiple patients across Dallas County. The RN collaborates with providers and other disciplines, recognizes and responds to emergencies, supports patients and families through education, and helps address patient social needs. The RN also documents care, maintains applicable licensure and certifications, supports compliance with relevant rules and standards, and contributes to staff training and departmental improvement.
