We are looking to add a Senior Navigator to our team. As the Senior Navigator you will be responsible for the oversight and delivery of Court Based Mental Health Navigator service in Oswego County. As the Senior Navigator you will work with local systems to coordinate voluntary care for justice-involved individuals to support their health, wellness, and recovery.

Requirements

  • Experience with the Psychiatric Services and Clinical Knowledge Enhancement Services (PSYCKES).
  • Experience in collaborating with the justice system.
  • Experience in developing and maintaining relationships with community based providers to support individuals in service.
  • Experience with data entry in an Electronic Medical Record
  • Obtain a valid health certificate to meet SJCS or current funding authority requirements as required.
  • Meet SJCS and the funding authority requirement of criminal background and record checks.
  • Successfully complete State Central Registry and Staff Exclusion List checks.

Nice To Haves

  • Bachelor’s Degree in Social Work, Counseling, Psychology, or related behavioral health or human services field, and a minimum of one year of professional experience in the mental health, criminal justice, or substance abuse field.
  • Senior Navigator is required to have at least one year of successful supervisory experience.

Responsibilities

  • Supervise and direct the work of the Navigator.
  • Facilitate communication, collaboration, and referrals with community stakeholders.
  • Provides training and consultation about navigator services and local resources.
  • Follows up on referrals, providing warm handoffs when possible.
  • Ensures data is collected and reporting is completed per County and OMH expectations.
  • Participates in OMH required meetings and provides input as required.
  • Conducts initial screenings with individuals
  • Connects individuals with services to address identified concerns.
  • Provides referrals, resources, and support in completing applications to address basic needs.
  • Connects with community providers on a regular basis to maintain referral and service pathways to support individuals in service.
  • Maintains documentation of services provided and reports such services through the identified documentation and reporting mechanisms.
