About The Position

We are looking to add a Navigator to our team. As the Navigator you will be responsible for supporting individuals in accessing community based mental health supports, and linkage to community based supports to support their health, wellness, and recovery.

Requirements

  • Experience with Psychiatric Services and Clinical Knowledge Enhancement Services (PSYCKES)
  • Experience in collaborating with the justice system.
  • Experience in developing and maintaining relationships with community based providers to support individuals in service.
  • Experience with data entry in an Electronic Medical Record.
  • Obtain a valid health certificate to meet SJCS or current funding authority requirements as required.
  • Meet SJCS and the funding authority requirement of criminal background and record checks.
  • Successfully complete State Central Registry and Staff Exclusion List checks.

Nice To Haves

  • Bachelor’s Degree in Social Work, Counseling, Psychology, or related behavioral health or human services field, and a minimum of one year of professional experience in the mental health, criminal justice, or substance abuse field.

Responsibilities

  • Conducts initial screenings with individuals.
  • Connects individuals with services to address identified concerns.
  • Connects individuals with community based mental health services.
  • Provides referrals, resources, and support in completing applications to address basic needs.
  • Follows up on referrals, providing warm handoffs when possible.
  • Connects with community providers on a regular basis to maintain referral and service pathways to support individuals in service.
  • Maintains documentation of services provided, collects client demographics, documents services provided, and assists with the preparation of report submissions.
