About The Position

This position serves as a Ryan White Case Manager responsible for client intake, financial assessments, updating demographic information in the HRSA CAREWare computer program, issuing URN cards, eligibility acuity, obtaining consents, rights & responsibilities, and Share of Cost assessments. All client eligibility documentation will be maintained in CAREWare. The Case Manager must ensure client confidentiality at all times, report security breaches immediately, and conduct face-to-face interviews to determine eligibility for Ryan White core services using state guidelines. The role also involves maintaining accurate client data in various state-approved programs, creating client charts, completing reports, coordinating care with the DOH-Volusia clinical team, assisting with health literacy education, maintaining knowledge of HOPWA program regulations, and referring clients to agency and local partners. Additionally, the position provides HIV education on medical adherence, transmission, and healthy lifestyle choices, participates in community events, and provides staff training in Ryan White Case Management and related subprogram areas. The ADAP program duties include providing social services, conducting interviews, entering client information into the ADAP database, enrolling clients in pharmaceutical assistance programs, assisting with paperwork, and providing education and referrals. The role also requires participation in quarterly conference calls and submitting monthly programmatic reports.

Requirements

  • Must be willing to physically come in the office to perform the duties & responsibilities of the positions.
  • Must have current authorization to work in the United States without employer sponsorship.
  • Knowledge of HIV/AIDS program.
  • Knowledge of social, economic or healthcare services.
  • Ability to communicate effectively verbally and in writing.
  • Ability to maintain confidentiality.
  • Ability to learn and communicate effectively, orally and in writing, in English.

Nice To Haves

  • 2 years experience with financial eligibility or case management.

Responsibilities

  • Serve as Ryan White Case Manager.
  • Conduct intake and re-determination processes for clients.
  • Perform financial assessments and update demographic information in HRSA CAREWare.
  • Issue URN cards, determine eligibility acuity, and obtain necessary consents, rights & responsibilities.
  • Complete Share of Cost assessments.
  • Maintain client eligibility documentation in CAREWare.
  • Ensure client confidentiality and report security breaches.
  • Conduct face-to-face interviews to determine eligibility for Ryan White core services.
  • Maintain accurate and current client data using programs like CAREWare, HMS, ADAP (Provide), FLMMIS, WAGES, and Change Healthcare.
  • Create client charts with current forms, signed releases, progress notes, medical data, and demographic information.
  • Complete monthly, quarterly, and annual reports for the supervisor.
  • Coordinate client care with the DOH-Volusia clinical team.
  • Assist with the implementation of health literacy education service programs.
  • Maintain current HOPWA program knowledge and adhere to state and federal regulations.
  • Identify and refer clients to benefits from the agency and local partners.
  • Provide HIV education on medical adherence, retention in care, transmission, and healthy lifestyle choices.
  • Participate in community events as requested.
  • Provide staff training in Ryan White Case Management, ADAP, Eligibility, and other applicable subprogram areas.
  • Provide social services to individuals or families needing ADAP services.
  • Enter pertinent client information into the ADAP database (Provide).
  • Enroll clients in Pharmaceutical Assistance programs.
  • Assist clients with required paperwork for ADAP.
  • Provide education and referral services to ADAP clients.
  • Participate in quarterly conference calls as requested.
  • Submit monthly programmatic reports to the program manager/coordinator.
  • Attend and participate in general staff meetings, staff development, team/staff meetings, and in-service training/continuing education.
  • Attend computer skills training classes as needed.
  • Maintain timely submission of EARS, timesheets, telephone invoices, and travel documentation.
  • Perform other duties as assigned.
