About The Position

This position serves as a Ryan White Case Manager responsible for client intake, financial assessments, demographic updates in HRSA CAREWare, issuing URN cards, eligibility acuity, obtaining consents, rights & responsibilities, and Share of Cost assessments. All client eligibility documentation will be maintained in CAREWare. The Case Manager must maintain strict confidentiality of client information and report any security breaches immediately. Specific duties include conducting face-to-face interviews to determine eligibility for Ryan White core services using state guidelines, maintaining accurate client data in various state-approved programs (CAREWare, HMS, ADAP, FLMMIS, WAGES, Change Healthcare), creating client charts, and completing monthly, quarterly, and annual reports. The role also involves coordinating care with the DOH-Volusia clinical team, assisting with health literacy education programs, maintaining knowledge of HOPWA program regulations, identifying and referring clients to applicable agency and partner benefits, providing HIV education on medical adherence, transmission, and healthy lifestyle choices, and participating in community events. Additionally, the position includes providing staff training in Ryan White Case Management and related subprogram areas. For the ADAP program, responsibilities include providing social services, conducting interviews, entering client information into the ADAP database (Provide), enrolling clients in Pharmaceutical Assistance programs, assisting with paperwork, and providing education and referral services. The role requires participation in quarterly conference calls and submitting monthly programmatic reports to the program manager/coordinator. Miscellaneous activities include attending staff meetings, development sessions, computer skills training, timely submission of administrative documents (EARS, timesheets, etc.), and performing other assigned duties.

Requirements

  • Knowledge of HIV/AIDS program.
  • Knowledge of social, economic or healthcare services.
  • Ability to communicate effectively verbally and in writing.
  • Ability to maintain confidentiality.
  • Must be willing to physically come in the office to perform the duties & responsibilities of the positions.
  • Must have current authorization to work in the United States without employer sponsorship.
  • Ability to learn and communicate effectively, orally and in writing, in English.

Nice To Haves

  • 2 years experience with financial eligibility or case management.
  • Experience in HIV.

Responsibilities

  • Serve as Ryan White Case Manager.
  • Conduct intake/re-determination and financial assessments for clients.

  • Assist with the implementation of health literacy education service programs.
  • Maintain current HOPWA program knowledge and adhere to state and federal regulations.
  • Identify and provide referrals for benefits from agency and local partners.
  • Provide HIV education regarding medical adherence, retention in care, transmission, and healthy lifestyle choices.
  • Participate in community events as requested.
  • Provide staff training in Ryan White Case Management, ADAP, Eligibility, and other applicable subprogram areas.
  • Provide social services to individuals or families needing ADAP services.
  • Enter pertinent client information into ADAP database (Provide).
  • Enroll clients in Pharmaceutical Assistance programs.
  • Assist clients with required paperwork for ADAP services.
  • Participate in quarterly conference calls as requested.
  • Submit monthly programmatic reports to the program manager/coordinator.
  • Attend and participate in general staff meetings, staff development, team/staff meetings, and in-service training/continuing education.
  • Attend computer skills training classes as needed.
  • Maintain timely submission of EARS, timesheets, telephone invoices, and travel documentation.
  • Perform other duties as assigned.
