Senior Community Residence Counselor (Full-time)
About The Position
The Charles Evans Center (CEC) is a recognized leader Federally Qualified Health Center, providing comprehensive medical, dental and mental health services. The center specializes in meeting the needs of adults and children with developmental disabilities as well as the underserved population. Our team-based approach puts patients at the center of their care. Using the highest standard of medical care; that reflects respect and commitment to our patients we give the patients the tools they need to achieve their best health. CEC is designed to enhance and promote the integration of medical and behavioral health disciplines to improve patients’ outcomes. They are seeking an experienced full-time Senior Community Residence Counselor who loves helping others, is interested in the field of psychology, and has a passion to do good and make a difference.
Requirements
- Bachelor’s degree
- Minimum one (1) year experience working in the field of Human Services
- Clean, valid New York State driver’s license
Responsibilities
- Ensure quality services are provided to each person supported through person-centered planning and maximize opportunities for integration with the community.
- Assist the Supervisor in monitoring and documenting the delivery of all services.
- Implementation of person supported behavioral support plans/strategies and documentation of person supported behaviors.
- Participate in the departmental on-call system including remaining on-call for the work week and participate in the weekend on-call system.
- Assist in the development and monitoring of staffing plans to ensure each person’s health and safety, including the possibility of providing direct care when necessary
